Please Fill out completely in order to receive credit.

*PLEASE NOTE THE FOLLOWING BEFORE SUMBITTING THIS FORM*:

  • Orders should be checked at the time of delivery unless it’s a key drop. Issues should be reported to the driver at the time of delivery and noted.
  • If the driver deducted the item from the invoice the credit should apply to your account soon after he closes out his delivery day.
  • Item will still be on the paper copy, but the cost of the case will be adjusted and not pulled with ACH. A credit does not need to be completed at this time.

Mispick: If you received an item you didn’t order (mispick), please submit clear photos of the product you received (YELLOW SYSCO LABEL & MARKINGS ON THE CASE) with your submission below.

Damaged: If you have damaged product, dented cans or spoiled product, please attach clear photos of the damaged product (YELLOW SYSCO LABEL, MARKINGS ON THE CASE & DAMAGED PRODUCT) with your submission below.

Short on Truck: Need to submit request for fresh items the same day as the delivery.
